Phacomorphic Glaucoma: Symptoms, Causes and Emergency Treatment

A very sudden loss of vision with a painful red eye is a symptom that should not be ignored in a person who has an advanced cataract. A cause may be phacomorphic glaucoma which is an ophthalmic emergency in which a puffy cataract obstructs the eye’s drainage of fluid. This in turn may cause a quick rise in intraocular pressure and put vision at risk.

Phacomorphic variety of lens induced glaucoma requires prompt evaluation. If treated late there may be damage to the optic nerve and other intraocular structures. Recognizing the warning signs may help patients to seek out eye care before they experience permanent vision loss.

What Is Phacomorphic Glaucoma?

Phacomorphic glaucoma is a category of secondary angle closure glaucoma that is related to an enlarged or swollen cataractous lens. As the cataract progresses the lens may grow in size. If the lens grows large enough the iris is pushed forward and causes the drainage angle in the eye to narrow or close.

This phacomorphic change causes poor drainage of aqueous humour. IOP may then go up quickly. As with a simple cataract this is not the case for phacomorphic glaucoma which presents with pain, redness and acute vision loss.

FeatureWhat May Happen
LensCataractous lens becomes swollen
Eye pressureIOP can rise significantly
Drainage angleBecomes narrow or closed
VisionSudden or severe reduction may occur
Eye appearanceRedness and corneal clouding may develop

What Causes Phacomorphic Glaucoma?

An in advanced stage cataract is what causes phacomorphic glaucoma. As the lens goes to a state of intumescence, or swelling, it takes up more space within the eye. This phacomorphic change causes the iris to move forward and reduces the depth of the anterior chamber.

Eventually in time aqueous fluid does not move and drains properly which in turn causes a pupillary block and angle closure which in turn causes phacomorphic glaucoma. Also people that put off cataract assessment or surgery as their vision worsens may be at greater risk of issues from an advanced cataract. But not all mature cataracts develop phacomorphic complications.

Symptoms of Phacomorphic Glaucoma

Phacomorphic glaucoma symptoms present very suddenly. A person may report intense eye pain, redness in the eye and rapid decline in vision. Also reported by some patients are colored halos around lights which is a result of corneal swelling from increased eye pressure. Also the affected eye may have a cloudy cornea or a different size pupil. These phacomorphic symptoms can also present like other acute eye conditions, which is why an ophthalmologist should make the diagnosis.

Warning SignWhy It Matters
Severe eye painMay indicate rapidly raised eye pressure
Sudden blurred visionRequires urgent eye assessment
Red eyeMay accompany acute angle closure
Halos around lightsCan occur with corneal swelling
Headache, nausea or vomitingMay accompany a sudden IOP rise

How Is Phacomorphic Glaucoma Diagnosed?

An ophthalmologist at the time of diagnosis reports that he does a detailed study of the eye in case of phacomorphic glaucoma. The doctor will test vision, measure intraocular pressure and look at the cataract, cornea, pupil and anterior chamber. A shallow anterior chamber and a cataract which is swollen also support this diagnosis. At times the doctor will study the drainage angle and the optic nerve if that is present which helps in the diagnosis. These reports also help in differentiating a phacomorphic attack from primary acute angle closure glaucoma and other causes of a painful red eye.

Is Phacomorphic Glaucoma an Emergency?

Yes. Phacomorphic glaucoma is a medical emergency which we see as a result of very high eye pressure that may persist for a long time. Also we have corneal damage, inflammation and in the worst case permanent vision loss which is a risk if treatment is delayed. At the first sign of acute eye pain, redness and reduced vision a patient should go to an eye hospital or ophthalmologist right away.

Emergency Treatment for Phacomorphic Glaucoma

Treatment of phacomorphic glaucoma usually has 2 main goals. We see first to reduce intra ocular pressure and control inflammation. Treatment may consist of pressure lowering eye drops or systemic medication which will be based on the patient’s condition.

Once we have brought the intraocular pressure and inflammation under control, cataract extraction will take care of the issue at root which in this case is the swollen lens. That is why cataract surgery is typically the definitive procedure for the phacomorphic mechanism. As far as timing of the surgery and what specific approach to use is concerned that is based out of the health state of the eye and what the ophthalmologist recommends. In the case of early intervention with phacomorphic glaucoma we see better results in terms of preservation of vision. Also the length of time which the pressure stays at very high levels is important as with time the optic nerve can be damaged beyond repair.

Can Phacomorphic Glaucoma Be Prevented?

Timely cataract assessment can reduce the risk of a cataract developing into serious lens related complications. As vision deteriorates gradually patients should not delay until the cataract is very advanced. As we age, regular eye exams become all the more important. Early identification of phacomorphic risk factors allows an ophthalmologist to put in place appropriate cataract management before a crisis occurs.

Conclusion

Phacomorphic glaucoma which is a serious but treatable result of late stage cataract. Presenting with sudden eye pain, redness and loss of vision this should be managed as an emergency rather than a routine cataract issue.

Prompt care for diagnosis allows doctors to manage eye pressure before permanent damage sets in. After stabilization, we address the primary phacomorphic cause with cataract treatment. Regular cataract exams and timely care are key to reduce the chance of reaching that emergency stage. For evaluation and proper management of cataract, glaucoma or sudden changes in vision patients may see an ophthalmologist at ASG Eye Care.

Frequently Asked Questions

1. Is phacomorphic glaucoma caused by cataract?

Yes. Phacomorphic glaucoma results from an enlarged cataract which in turn causes pupillary block and closure of the eye’s drainage angle.

    2. Can phacomorphic glaucoma cause permanent blindness?

Untreated phacomorphic glaucoma may produce permanent visual damage if the intraocular pressure remains high for a long enough time which in turn damages the optic nerve and other eye structures

      3. Is surgery necessary?

The immediate issue is to reduce eye pressure and control inflammation. Cataract removal which takes out the swollen lens responsible for the phacomorphic mechanism is usually the definitive treatment

        4. Can eye drops cure the condition?

Eye drop and other medications may reduce intraocular pressure, but they are unable to reverse the condition of a swollen cataractous lens. In terms of treatment, we see that for phacomorphic glaucoma, medication does not fix the root cause.

When should I visit an eye hospital?

Seek immediate eye care if you present with severe eye pain, redness, sudden blurred vision, halos, headache, nausea or vomiting which may occur in association with cataract.
