Macular Pucker: When an Epiretinal Membrane May Need Surgery

Changes that occur in central vision may present as difficulties in everyday tasks like reading, driving, or recognizing faces. A cause may be a macular pucker which is the growth of a thin tissue layer over the macula. This tissue is what is known as an epiretinal membrane.. What Is a Macular Pucker?

How Is an Epiretinal Membrane Diagnosed?

OCT is also able to show epiretinal membrane, retinal wrinkling, and changes in macular thickness. Does Every Macular Pucker Need Treatment?

No. Many patients report only mild symptoms which do not require at this time macular pucker treatment. For stable and functional vision regular eye exams may be enough.

The National Eye Institute reports that in mild cases treatment is not required. When vision distortion which in turn affects day to day activities occurs, that is when surgery becomes a recommendation.

When May Surgery Be Considered?

An ophthalmologist may present ERM surgery as an option in the case of an epiretinal membrane which causes large scale visual disturbance or reduced central vision. Also if there is an issue with reading, driving, working or performing fine tasks this may play a role in that decision. 

In a typical surgical approach we see vitrectomy used. At surgery the ophthalmic surgeon removes the vitreous gel and gently peels the membrane from the retina. This process in turn releases the traction which causes macular distortion.

The aim of ERM surgery is to decrease distorsion and improve useful vision. Also improvement may present slowly as opposed to immediately. The degree of improvement also varies between patients. 

For that which is at hand, macular pucker treatment must tailor to the patient’s symptoms, retinal findings, visual needs and that which pertains to the health of the whole eye.

Can Macular Pucker Come Back After Surgery?

An epiretinal membrane may present again after surgery which is however not the case for all patients. Routine follow up exams allow the ophthalmologist to see the retinal healing progress and to notice any new changes. 

Patients that are looking at ERM surgery should go over with their retinal specialist what to expect in terms of benefits, limitations, recovery, and also the possible complications. It is also true that the decision regarding macular pucker treatment should come after a full clinical assessment.
